HealthPathways Melbourne is a free, web-based portal with relevant and evidence-based information on the assessment and management of common clinical conditions including referral guidance.
The pathways have been designed for use during consultation and are jointly developed through collaboration between hospital clinicians and community clinicians.
The pathways are written by GP clinical editors with support from local GPs, hospital-based specialists and other subject matter experts.

Eastern Melbourne PHN and North Western Melbourne PHN are working together to implement HealthPathways Melbourne, in collaboration with The Royal Melbourne Hospital, St Vincent's Hospital Melbourne, Eastern Health, The Royal Women's, and Western Health, drawing on expertise from each organisation and maximising the benefits to GPs.
