What is HealthPathways?
HealthPathways Melbourne is a free, web-based portal with relevant and evidence-based information on the assessment and management of common clinical conditions including referral guidance.
The pathways have been designed for use during consultation and are jointly developed through collaboration between hospital clinicians and community clinicians.
The pathways are written by GP clinical editors with support from local GPs, hospital-based specialists and other subject matter experts.
How to access HealthPathways Melbourne
If you are a health professional in Melbourne’s north western or eastern region, you can access the site by requesting the username and password from the HealthPathways Melbourne team - email info@healthpathwaysmelbourne.org.au.
Who is involved in HealthPathways Melbourne
HealthPathways Melbourne is a collaborative program run by the Eastern Melbourne PHN (EMPHN), and North Western Melbourne PHN (NWMPHN), and is supported by participating primary care clinicians and consumers, Eastern Health, The Royal Melbourne Hospital, St Vincent’s Hospital Melbourne, The Royal Women’s Hospital, Western Health, The Royal Children’s Hospital, Northern Health, Monash Health, Mercy Health, Austin Health, Djerriwarrh Health, The Royal Victorian Eye and Ear Hospital, Alfred Health, Peter MacCallum Cancer Centre, community health, council, and other health organisations in both the EMPHN and NWMPHN regions.
Other hospitals will be invited to participate in subsequent phases of the program and/or to contribute specific clinical knowledge and expertise.
